Week 1 discussion

This class uses a different approach to discussions. Each
student is assigned to one of four groups (Group A-D) and each group has 2
weeks during the 8 week class in which they are responsible for the initial
response postings, and the other 6 weeks the groups respond to others’
postings. The responses will be done by each member of the group; this is not a
“group project,” but just a way to divide the class. To determine
which group you are in, go to My Tools > Groups.

Weekly Assignments for Groups

Group A: Each member posts main responses in Weeks 1 and 5;
replies to other students’ main responses in weeks 2, 3, 4, 6, 7 and 8.

Group B: Each member posts main responses in Weeks 2 and 6;
replies to other students’ main responses in weeks 1, 3, 4, 5, 7 and 8.

Group C: Each member posts main responses in Weeks 3 and 7;
replies to other students’ main responses in weeks 1, 2, 4, 5, 6 and 8.

Group D: Each member posts main responses in Weeks 4 and 8;
replies to other students’ main responses in weeks 1, 2, 3, 5, 6, and 7.

Electronic Health Records

All technology advancements, by their very nature, offer
both benefits and problems. Electronic Health Records (EHR) is no exception.

The following article outlines a number of the good, bad and
ugly issues with EHR.

http://www.beckershospitalreview.com/healthcare-information-technology/electronic-health-records-the-good-the-bad-and-the-ugly.html

Given all of the positive and negative aspects associated
with the use of EHRs, how do we decide if the good aspects outweigh the bad? If
we decide to move forward with using EHRs, how do we mitigate the negative
aspects?

For this week, each member of Group A should create a main
posting by Thursday midnight, and each member of Groups B, C, and D should
respond to at least two class members by Sunday midnight. Since this is a
“discussion,” you should check back to see if I or anyone in the
class have a question for you, or make a comment to which you wish to respond.

Please review the Grading Rubric located in the Discussions
area to see how discussion participation will be graded, and note that, if you
are doing the primary response, you need to reply back to classmates who post
in your thread, and if you are in the groups replying, “more than 2”
replies are needed to receive full points (a grade of “A”).

Week 2 discussion

Group B: Drawing from your own experience, select a process
(a set of specified steps to accomplish a task) used at your place of work or
in your interaction with a health care organization that you would like to see
improved. Explain why you picked that process. Explain how you would go about
improving it. Who should be involved with you? What are some of the questions
you should ask about the current process? How will you know if the process was
actually improved?

First, we need to be sure you can identify a process; many
students have difficulty with that, so refer to your textbook and other
readings. I am also interested in the method to be used to improve the process,
not a solution. (For example, I am not looking for something like: “The
clinic where I work is new and has few patients so far. We should use social
media to advertise more.” The discussion here is about identifying a
process and how the clinic would go about deciding how to improve that process,
who should be involved, what should be considered, and what steps should be
taken.) You should employ the techniques discussed in class or those that you
find in your research. Keep in mind that outside resources strengthen your
responses.

Remember, members of Group B should create a posting in response
to the questions above by midnight Thursday. These postings need to thoroughly
respond to the questions. Then members of Groups A, C and D will respond to at
least two students’ postings with comments, questions and further discussion.
(Remember “more than 2” substantive postings are required for an
“A.”) If questions are posed, they should be responded to by the
original poster. Let’s have a great discussion and some interesting examples to
take a look at for improving processes!

Your first assignment, Stage 1, requires you to analyze a
process and suggest ways it can be improved. This discussion is to help prepare
you to successfully complete that assignment.

Week 3 discussion

Group C: There are many tools that take vast arrays of data
and create information in a form that helps us better understand what the data
tells us. One group of tools are called data visualization tools. Read the
article below and select one of the methods of data display. Explain how that
particular display helps us understand the data better. If you can, it would be
helpful to copy and paste the data display into your posting, or provide a link
to it. (Be sure to cite your source correctly.)

http://www.healthdata.org/results/data-visualizations

Group C’s main responses are due by Thursday. Groups A, B
and D’s replies to classmates’ postings are due by Sunday night. (Remember
“more than 2” substantive reply postings are required for an
“A.”)

Week 4 discussion

It seems like we cannot go a week without hearing of a major
incident of hacking. Some recent large issues included incidents effecting
Target, Google, Facebook, Playstation, and University of among others.
All of these organizations believed that they were safe until they found out
that they were not. In fact, most of them did not know that they were hacked
until the Government picked up messages containing the hacked information being
transmitted outside of the country. The following video discusses one such
attack which included a significant healthcare player (Merck).

The healthcare industry, as the rest of our society, trusts
the information that is contained in our system. The following video presents
the story of a person whose identity was stolen, a hospital that believed that
they had treated her, and the repercussions that she experienced due to this
error.

https://www.youtube.com/watch?v=Y3aPB5Afdks

This last issue addresses medical devices. Many medical
devices are computers which are controlled remotely. There have been instances
of these devices being hacked, adversely affecting the patient. The following
video discusses the issue.

This problem is so serious that, when Dick Cheney was Vice
President and received a pacemaker, the Secret Service determined that his
pacemaker would not have the ability to be remotely managed. The following
video discusses the specific issue with the Vice President.

At this point, we have to expect that there is a possibility
of these types of attacks whether it is stealing personal information, stealing
a person’s identity, or remotely hacking into a medical device.

Group D: What can we do to protect ourselves and our
patients?

Remember, members of Group D should create a posting in
response to the questions above by midnight Thursday. These postings need to
thoroughly respond to the question. Then members of Groups A, B and C will
respond to at least two students’ postings with comments, questions and further
discussion. (Remember “more than 2” substantive postings are required
for an “A.”) If questions are posed, they should be responded to by
the original poster.

Week 5 discussion

Review this week’s reading on “System Quality
Attributes” at

https://msdn.microsoft.com/en-us/library/ee658094.aspx

Group A: Select one of the quality attributes listed and
explain what it means, why it is important in an EHR system implementation, and
what the Family Clinic will need to do to ensure that attribute for their
system.

Group A’s initial responses are due by Thursday, and replies
from Groups B, C and D to at least two classmates are due by Sunday. (Remember
“more than 2” substantive response postings are required for an
“A.”)

Week 6 discussion

As identified in the Family Clinic Case Study, there
are several clinical and administrative processes that could us improvement,
including: scheduling patients, checking patients in, ordering laboratory
tests, submitting prescriptions to pharmacies, ordering office supplies,
billing insurance companies, etc. Separate IT solutions could be implemented
for each of the processes. However, you know that if you could combine these
into an enterprise system, you would be able to significantly improve the
operations of Family Clinic.

Group B: You begin your analysis by answering the following
questions:

What opportunities would be missed if each area (scheduling,
check-in, supply ordering, billing, etc.) maintained its own “stove
piped” data? (Provide some examples.)

What benefits will the Family Clinic realize if you
implement an enterprise-wide system? Identify how Dr. Martin as the overall
owner and manager will benefit from having additional information available.

What types of decisions that he makes would be improved if
he had all this information in one place?

Group B initial responses are due by Thursday, and replies
from Groups A, C and D to at least two classmates are due by Sunday. (Remember
“more than 2” substantive response postings are required for an
“A.”)

Week 7 discussion

First, review the ‘triple constraints’ of IT Project
Management as covered in this week’s reading “What is Project
Management?”

Group C: Please address the following 3 questions:

What do you think is the most important of the triple
constraints of project management for the Family Clinic as it implements
an EHR system?

Explain briefly why you selected that constraint.

Discuss how you would ensure that this aspect was well
addressed to maximize the chance for a successful implementation at the
Family Clinic.

Group C’s main responses are due by Thursday. Groups A, B
and D’s replies to classmates’ postings are due by Sunday night. (Remember
“more than 2” substantive reply postings are required for an
“A.”)

Week 8 discussion

DQ1

With everything that we have covered so far in class, and
what we read and see in the world around us, it is obvious that major changes
are happening all around us. And, changes seem to be occurring at an ever
increasing rate. The following video is Microsoft’s idea of some of the changes
that we can expect to our lives due to changes in technology.

As part of these changes, there will be changes in how
healthcare is delivered to the society. Some of these technologies are
beginning to become available, although they are not in common use. These
advancements are likely to result in cheaper, easier access to healthcare;
continuous monitoring; and different paradigms in the healthcare industry. The following
video presents Microsoft’s idea of some of the changes that we can expect in
the healthcare industry.

This brings us to an interesting issue. Most, if not all of
us, are taking this curriculum in order to advance in our career in the
healthcare field. With the anticipated changes to the delivery of healthcare,
there will be new jobs that currently do not exist, as well as changes to
duties of existing jobs and some jobs eliminated.

As far as new jobs, the following video reviews jobs in
Healthcare Information Technology.

Group D: How do you believe that your current or future job
within the healthcare industry will change…and how will you respond to that
change? (If you are not in the healthcare industry, pick a position and answer
the question from that perspective.)

Group D’s initial responses are due by Thursday and Groups
A, B and C’s replies to at least two classmates are due by Sunday. (Remember
“more than 2” substantive reply postings are required for an
“A.”)

DQ2

I would like for each of you to think back over the class
and share with us one new and / or important concept that you learned during
this class AND, if applicable, how you may be able to apply this concept. Also,
if there are any recommendations to improve the next offering of the course
please let me know either in this forum or via email. You can also add any
areas of improvement and / or things you liked about the course (so I know what
to keep) in the end of course survey – which I hope everyone will complete.
Your survey responses help me improve the next offering of the course. Thank you
for a great semester.

Your response counts as part of this week’s participation
grade, and you are NOT required (but are invited) to respond to your
classmate’s postings. You can receive full-credit for a good initial post!

I look forward to reading your comments!
