1. Description of the patient’s health issues, diagnoses, and treatment plan.

2. Description of the interdisciplinary team that will participate in this care.

3. Can the CCCR model be used in managing this patient’s care? How so? If not, can you name another model to help in managing this patient’s care?  

4. What is the cost of managing this disease? Are there resources available for the patient and family to help with costs and other factors such as commute and etc?

5. What is your role as the APRN in this team?

6. As the APRN, how can you evaluate if the care management is successful?

7. As the APRN, how can you evaluate if this patient needs a higher level of care?

8. Are there any barriers or challenges that you foresee for this patient? How can you overcome these barriers?

· The body of your paper should be a three to four pages long. This does not include the title page and reference page. Papers that are more than four pages long will not be accepted. Incorporate information from the course text plus a minimum of three (3) scholarly references published within the last five (5) years. Textbooks are not to be considered scholarly references.

· Use current APA Style to write your paper and to cite your sources. Your source(s) should be integrated into the paragraphs. Use internal citations pointing to evidence in the literature and supporting your ideas.
