Topic: Nursing Care Plan for the Patient with Stroke

Purpose: This assignment will enable students to identify the problems of the client with stroke and determine appropriate nursing interventions for the proposed nursing care plan.

Case Scenario:

John, aged 44, was a bus driver with good past health. He could not get out of the bed at the usual time wake up time of 7 a.m. on 8.8.2021. He found that there was no power over the right side of his body, slurred speech, and facial deviation. His wife called an ambulance to send him to the nearby Accident and Emergency Department (AED). The doctor in AED suspected that he had a stroke.

His vital sign was as follows: BP 172/82 mmHg, Pulse 80/mins, SPO2 96% (Room Air) at the AED. ECG showed sinus rhythm. An intravenous cannula was inserted and blood was taken for Completed Blood Picture (CBP), Renal Function Test (RFT), Liver Function Test (LFT), Clothing (INR), and Spot Glucose. His Histix showed 5.8mmol/L. The limb power in the left hand and left leg were 5/5, but no power in the right hand and right leg 0/5. Computed tomography (CT) showed ischemic stroke. The neurology doctor was consulted for the administration of recombinant tissue plasminogen activator

(tPA). Owing to the uncertainty of the onset time of stroke, the neurology doctor did not recommend administering tPA. John was admitted to the medical unit for further management.

 

The anti-hypertensive drug, cholesterol, and antiplatelet drug were prescribed during the time in the medical unit. John stabilized on Day 2. The limb power over the left hand and left leg were 5/5, but the power in the right hand and right leg was improved to 1/5. His vital sign was BP 132/82mmHg, Pulse 80/min. He wanted to regain his power over the right side of his body and resume normal life. After the case conference, John was put into the list of a rehabilitation program. He was transferred to the rehabilitation unit the next day.
