DESCRIPTION
The Wellness Primary Care Trust (WPCT) consists of four medical practices, each with different numbers and types of staff both medical and non-medical.
Each Medical Practice records information in an Oracle database.

* A patient’s information will only be recorded if there has been an appointment. An appointment for a patient initially identifies the patient’s problems. These problems are recorded in the Diagnosis table. ( For example, a patient might be admitted following an accident could have sustained a Broken Right Arm, Broken Right Leg and Head Injuries)

* A Diagnosis can only be made following an appointment, and the Date and Time is recorded as the Date and Time of the appointment.

* The Referred attribute indicates whether this is an emergency appointment or the patient has been referred by their G.P. (i.e. its domain of values will be “EMERGENCY”, “REFERRED”) to a hospital.

* A General Practitioner is associated to only one Doctor’s Practice which may consist of more than one doctor.

1. Use a simple Entity Relationship Diagram (ERD) to show the Hampton Medical Practice database design. The diagram should use the type of labels and notation taught in Computing Modules in your undergraduate course ,Include The entity relationship matrix.
Create it Using Oracle Modeller Appropriate relational database rules for cardinality and suitable constraints should be included. You must include all primary keys, foreign keys and any additional constraints that are applicable
