Over the next few weeks you will be working on a summative community health promotion evaluation.

For this project you will assess the needs of a particular community and design a community program for health promotion.

You will also develop implementation strategies and intervention methods to address these community needs.

Finally you will evaluate the program planning process and revise your project as needed.

This week you will design your project with your chosen theory or model in mind. Although your project presentation will not be due until week 7 you will be building up to that final presentation over the next weeks.

Please focus on the needs of your community.

Write a summary of health needs to identify a population or group in need of health promotion and education.

What resources are available to help reduce these issues?

Write a 1-2 page paper in APA format paper on the following topic.

From the holistic therapies you are aware of, identify one that interests you.

How could you integrate this holistic modality into your community project?

Would it be applicable across variables such as age, culture and disability?

If you are unfamiliar with any holistic therapies, please do a quick web-search!

Reflect on the community needs you have identified, and identify a health promotion topic you select as the focus of the community program project.

Consider barriers within the community that might interfere with improving health behavior outcomes.

Consider whether there are community resources designed to address the community needs you identified and help patients overcome barriers and minimize risk behaviors (i.e., health promotion media, handouts, support groups, etc.).

This week’s assignment will serve as the final project. It is a comprehensive exercise designed to integrate the topics you have learned during this course into a personal health promotion community project.

Community health programs are important for promoting healthy behaviors in individuals, families, and communities. By implementing health programs that target specific community needs, providers can influence patient behavior changes and improve long-term health. As you finalize your community health program plan, consider the various factors that contribute to the effective design of health programs. Keep in mind that community needs and resources should drive your planning, implementation, and evaluation.
