    Oh gosh, I guess probably the last time I had to go to the hospital for it, which was when I was sixteen. I definitely haven’t had one since high school.

    Yeah, I’m allergic to penicillin.

    What happens with Penicillin

    I don’t remember that well because the last time I had penicillin I was pretty little. . . I think I got a rash, like, hives. The doctor told me not to take it again after that.

    What over the counter medication are you taking

    Usually I’ll take Tylenol sometimes for a headache, and I take Advil for cramps. Lately I’ve been taking the Tylenol more than usual.

    Ms Jones who is african american pleasant female who presents to the clinic with c/o headache and neck stiffness She was in a slow moving car accident last week, and her symptoms just started two days ago. She was wearing her seat belt. She has a dull headache at the crown of her head and the back is at the back of her neck. She was no loss of consciousness. She takes tylenol which helps, with the pain 3/10. She feels her neck swollen, but has been resolving recently. Moving her neck hurts. ROS: She denies any fatigue, wt loss, fever or chills. Head: NO trauma, or LOC or dizziness,. Eyes: she wears reading glasses, and gets blurry when she reads to much. Ears: no hearing loss or ringing of her ears, no vertigo or earache. Denies any neck stiffnes. Musculoskeletal: denies problems with her ROM Denies any neurological disease or problems. No family hx of neurological disease or problems. Medical hx: Diabetes: no meds Asthma: Medications: Albuterol, proventil, tylenol and advil Allergy med: PCN: hives, and rash

    HPI: Ms. Jones presents to the clinic complaining of a headache neck stiffness that started 2 days after she was in a minor fender bender. One week ago she states that she was a restrained passenger in an accident in a parking lot and estimates the spee be approximately 5-10 mph. She and the driver did not seek emergent care and felt fine after the accident. Two days later, however, she developed a bilateral temporal dull ache accompan by neck ache. She states that she feels as though her neck may slightly swollen as well. She did not lose consciousness in the accident and denies changes in level of consciousness since tha time. She states that she gets a headache every day that lasts approximately 1-2 hours. She occasionally takes 650 mg of over counter Tylenol with relief of the pain. She denies known associa symptoms. Review of Systems: General: Denies changes in weight, fatigue, weakness, fever, chills, and night sweats. • Head: Denies history trauma before this incident. Denies current headache. • Eyes: Sh does not wear corrective lenses, but notes that her vision has be worsening over the past few years, but no acute changes. She complains of blurry vision after reading for extended periods. De increased tearing or itching. • Ears: Denies hearing loss, tinnitus, vertigo, discharge, or earache. • Nose/Sinuses: Denies rhinorrhea Denies stuffiness, sneezing, itching, previous allergy, epistaxis, o sinus pressure. • Musculoskeletal: Denies muscle weakness, pai difficulties with range of motion, joint instability, or swelling. • Neurologic: Denies loss of sensation, numbness, tingling, tremor weakness, paralysis, fainting, blackouts, or seizures. Gait steady with continuous, symmetric steps. Sensat intact to bilateral upper and lower extremities; sense of extremity position intact. Stereognosis and graphesthesia intact bilaterally.

    Assessment

    Headache and Neck pain due to s/p slow moving MVA Acute post-traumatic headache following low-speed MVA where Jones was a restrained passenger

    Plan

    Try Motrin 800mg po every 8 hours prn for headache Offer ice or heat to see if it helps with pain Educate on Warning signs: report any increase headache or worsening headaches, LOC, dizzinees or vomiting Education on exercises for ROM of neck Report to ER if headache is severe, LOC, nausea and vomiting, dizziness. Advise to have someone with her to watch her for the next 2 days if worsening symtoms. Telephone appt in 2 days to follow up with symptoms. Consider neuro consult if symptoms don’t resolve CAT scan if sx worsens or no improvement

    Encourage Ms. Jones to continue to monitor symptoms and repo any increase in frequency or severity of her headaches. • Initiate treatment with ibuprofen 800 mg by mouth every 8 hours as nee with food for the next 5 days. • Ms. Jones can also use adjunct therapy of topical heat or ice per comfort TID-QID. • Educate on stretches for upper back and neck. • Educate on when to seek emergent care including the worst headache of her life, acute changes in vision, hearing, or consciousness, episodes of nause vomiting associated with headache, or numbness, tingling, or paralysis of new onset. • Ask Ms. Jones to call the office in two d to discuss symptoms. If no decrease in symptoms, order a computerized tomography scan or magnetic resonance imaging

    Well, I’ve been taking Advil for it. Otherwise the only thing that really helps is lying flat on my back.

    I’ve been taking them every five or six hours, like it says on the bottle.

    It definitely feels worst when I’m sitting up. Moving around can make it hurt more, too.

    Without medication, it depends on what position I’m in, but I’d say probably it averages about 5 out of 10. (Clarified to to rate my back pain on a scale.)

    When the Advil’s just kicking in the pain goes down to a 2 or 3 out of 10, I’d say.

    Are you having any joint pain

    No, my joints feel fine.

    Are you able to stand up

    It actually feels a little better when I’m standing than when I’m sitting.

    Have you had history of muscle soreness before

    Sore muscles aren’t usually a problem for me, but I think it’s this muscle in my back that is causing me so much pain. [AUDIO COMING SOON]

    May I ask what do you do at work

    Well, since being promoted to supervisor, I’ve been in charge of scheduling other employees, taking inventory, and placing orders for supplies. I’m still on the floor a lot though, dealing with customers and that sort of thing. It’s mostly a lot of standing and walking around.

    Well, I guess it’s the muscle in my back that’s hurting.

    This is a 28 yo pleasant african american female. She is the primary source of history and offers information freely. she speaks clearly and coherent and maintains contact throughout the exam. She presents to the clinic with c/o back pain for 3 days. Pain is 5/10, takes advil and pain reduces to 2-3/10. Her pain in her lower back and upper buttocks. Gets worse when she is laying down.     HPI: Ms. Jones presents to the clinic complaining of back pain th began 3 days ago after she “tweaked it” while lifting a heavy box while helping a friend move. She states that lifted several boxes before this event without incident and does not know the weight the box that caused her pain. The pain is in her low back and bilateral buttocks, is a constant aching with stiffness, and does n radiate. The pain is aggravated by sitting (rates a 7/10) and decreased by rest and lying flat on her back (pain of 3-4/10). The pain has not changed over the past three days and she has treat with 2 over the counter ibuprofen tablets every 5-6 hours. Her current pain is a 5/10, but she states that the ibuprofen can decrease her pain to 2-3/10. She denies numbness, tingling, mus weakness, bowel or bladder incontinence. NOTE: Grassroots Activism Project requires you to look around and identify an issue of
concern in your own community, develop ways to address the concern, and then take
action! You’ll then be able to consider barriers to action and how you can use your

have guidance along the way and opportunity to get a restart if you stumble.
COVID-19 UPDATE: Although you may be doing an infographic, PowerPoint, or similar
alternative “action” (since many of you cannot complete the action you proposed earlier
due to concerns over COVID-19 and current safer-at-home orders), you still need to
complete the essay portion of this assignment. Your infographic, Ppt, etc is your
“action” now, but the essay is the context for your action.
In this stage of our Grassroots Activism Project ( assignments, we will engage in grassroots
activism by “doing” something and documenting it! You should plan on about 3-5 hours of time
and effort total. Your assignment in the Action Stage – besides the actual implementing your
grassroots activism plan – is to complete an “Addressing Injustice Report” to reflect on your
experience and action. You will submit an essay addressing these questions: What did I do
to address my chosen social injustice? What barriers or frustrations did I face, and how did I
overcome them? What would I do differently next time I seek to address social injustice? To
what degree did my grassroots activism make a change? You should also include photos and/or
other materials that document your experience. That way we know that you actually completed
the project.
REMEMBER: This doesn’t have to be writing a whole new essay! You may take aspects of your
earlier assignment and add to it in a way that builds on it. Be sure to answer the posted
questions specifically and with enough detail and explanation that anyone off the street could

GAP assignment #1
The admission of students with athletic ability is common in the United States. A study by Hurwitz
revealed the odds of admission for athletic or legacy students was 15.69 times the odds for other students
(486). About 40 percent of sports in college classes are reserved for legacy students, and elite colleges
give up to 24 percent of undergraduate positions to legacy students (Bergman para 5). Some studies reveal
that legacy criteria can substitute for several dozen points on the SAT (Ordway para 3). The high level of
injustice towards academically superior students who are denied college positions for lack of superior
athletic ability can be understood in the context of competitive advantage theory. The theory stresses on
the maximization of economies of scale, which is applicable mostly in business (Wang 33).In sports, it
could mean that colleges strive to present themselves as capable of nurturing talents. Thus, it might
appear worthwhile to pay for high premium prices because the product is multifunctional and
well-rounded. Also, students and sponsors can discern the opportunity cost of attending a sports-oriented
college or a non-oriented college.
The issue of injustice meted towards academically strong students is related to the concepts discussed in
class. It relates to discrimination based on social-economic status and race. Indeed, reserving and
providing positions to students with athletic ability while denying the same positions to students with
higher academic qualifications is discriminatory. Besides favoring students from superior economic
backgrounds who can nurture their own talents using their personal fundings, discriminative college
sports awards students whose racial characteristics facilitate athletic prowess. The practice is wrong, as
discussed in class, because it propagates a culture of injustice, eliminates fairness, and encourages
institutional discrimination, stereotypes, and segregation in educational systems.
I will address the raised issue by holding discussions with my college registrar and sports head in the
coming week. I will book appointments, and when I meet them in their offices, I will determine the exact
number of student-athletes, the college admission criteria, and the equivalent of the edge given to a
student-athlete in terms of SAT or other scores. I will then ask the college to follow up on the athletic
students as well as the students denied admission positions to determine how the students fair in their
studies and the institutions in which they get enrolled. By so doing, the college can track the impact of its
policies even as it revises the admission criteria and relaxes athletic ability preferences.
Instructor comment :
This is an interesting topic. So are you focusing on the socioeconomic inequity that results in
legacy students essentially buying their way into schools? I am not sure what you mean when
you stated that, “college sports awards students whose racial characteristics facilitate athletic
prowess” as there is nothing that I am aware of that differentiates athletic ability based on race.
This is a potentially problematic perspective, as it reproduces racial hierarchy, so be careful with
your wording. If you are going to address unfair policies in college recruiting, that makes sense
but you will need to connect it strongly to class and the wealth gap in order for it to work with
this class. Additionally, part of your action is asking the school to compete research. That is not
feasible. Your action about meeting with people, do you think that those people have the ability
to change policies? Might there be other factors like board funding at work here? I don’t think
meeting with people is an effective use of your time for this project. How else might you raise
awareness about this issue? Could you hold an info-night for your family and friends and raise
awareness about how classism effects college recruiting, particularly athletics? Keep thinking
about this project, and if you need more help please reach out to us.

The assignment consists of three parts:
Part 1: Data Analysis
When conducting the analysis, you will apply techniques from descriptive analytics, visualisations,
probabilities, and confidence interval calculation. Hence, you will use various tables, graphs, and summary
measures. When exploring data, we often produce more results than we eventually use in the final report, but by investigating the data from different angles, we can develop a much better ‘feel’ for the data: a deeper understanding of the data.
 Always ensure that you consider relevant modelling assumptions such as considerations of the level of measurement of the variable, the method of sampling, the shape of the population distri- bution, and the sample size.
 The analysis section you submit should be on Q1 to Q5 sheets of the Excel file.
 Where possible, it is always useful to produce both numerical and graphical statistical summaries as
sometimes, something is revealed in one that is not obvious in the other.
 Your analysis should be clearly labelled and grouped around each question.  Poorly presented, unorganised analysis, or excessive output will be penalised.
Part 2: Email
You are required to reply by email, detailing all essential information and relevant conclusions from your data analysis. You are allowed no more than 2 pages to cover your written conclusions. Please use the font size 11 (Arial) and leave a margin of 2.54 cm.
Please consider the following dot points very carefully.
 Keep the English simple and the explanations succinct. Avoid the use of technical statistical jargon.
 Your reader will not necessarily understand complicated statistical terms, thus your task is to convert your analysis into plain, simple, easy to understand language.
 The email is to be written as a stand-alone document. Thus, you should not have any references in the email to your analysis, nor should you include any charts and tables in your email.
 Use an email format for your reply. That means the email heading (e.g. To: From: Subject 🙂 should be included, the recipient should be addressed at the beginning and the signature or name of the sender should be included at the end.
 When composing your reply, make sure that you actually answer the questions asked.
 Do not copy the questions in the email.
 Sequentially number your answers in both your email and your analysis (1, 2 …5) to match the email.  Include a simple introduction at the start of the email and a summary/conclusion at the end.
 Marks will be deducted for the use of technical terms, irrelevant material, poor presentation /

FINAL PROJECT (30% of Final Grade)
Final Paper Due: 5 pm, Thursday, May 14th, 2020
For the final POLS 302 Research Paper, you will examine the degree of congruence between
public opinion on a major policy issue and election outcomes in terms of congressional and/or
presidential results, using an ANES dataset from the 2004-2016 period.
Your paper, thus, will include your summary of results, cross-tabulations, polls and statistical
analysis presented by others (such as polling conducted by news outlets and academic
organizations) AS WELL AS your own original research. You should address the following
questions:
• What were the patterns of public opinion on the issue during the period under study?
• To what extent were the patterns in the time period of your study similar to or different
from opinions at some previous point in time (up to twenty years earlier)?
• How did opinion on that policy issue differ among various groups? Look for opinion
differences based on categories such as age, race, gender, region, religion, place of
residence, education, partisanship, ideology or any other quality that might be relevant?
• If relevant, what were the policy options advocated by various candidates and/or
interests?
• What policies or points of view seemed to prevail on Election Day?
• Thus, to which segments of public opinion were President and Congress responding?
• How did voters respond to policymakers’ positions on a given issue or set of issues?
To answer these questions, concentrate on using a more recent of data such as ANES 2004 or
2008, 2012, or 2016. You should FORMAT / ORGANIZE the paper per the following:
• Title Page (1 page, followed by a blank page)
• Table of Contents (1 page)
• List of Tables and Figures (1 page, including Labels, or Title, for each chart, graph, etc.
with page numbers, followed by a blank page)
• Abstract (1 page, one paragraph summary of your research question and brief findings,
followed by a blank page)
• Introduction (1 to 2 pages in length, introducing your research, why the question is of
interest, and a short discussion of where the research falls in Political Science)
• Previous Research / Literature (3 to 5 pages in length that details past significant research
and scholarship on your particular question)
• Theory (2 to 4 pages in length that details how you conceptualize previous approaches
AND integrate your own view into a new theoretical contribution)
• Research Question / Hypothesis(es) (1 to 2 pages in length that includes your predictions
about the DV based on the interaction of the IVs. This is both a formula AND a
description of each variable.)

• Methodology (1 page in length. A discussion of how you operationalize the IVs and the
statistical methods used to answer your hypothesis(es).)
• Findings (3 to 5 pages in length. What did you find, usually includes tables and
discussion of the significant findings within those tables.)
• Discussion / Implications (2 to 4 pages in length. A discussion of, “What is significant
about your question and based on your findings, why does that matter?”)
• Conclusion (typically 1 page, followed by a blank page)
• Appendix (Tables, supplementary information, followed by a blank page)
• Bibliography /Works Cited (should mostly – almost exclusively – be comprised of
Political Science journal articles, and few, if any, books.)
PAPER LENGTH & FURTHER GUIDELINES
The minimum accepted length of the WHOLE paper is 30 pages. The final paper, including all of
the pages listed above (including the blank pages), should total between 30 and 50 pages.
EVERY page should be paginated. The paper will be typewritten, double-spaced, using 12 point
Times New Roman font. The paper should be stapled once in the upper left-hand corner with no
plastic covers.
You must document every source of information you rely upon, including identifying each
statistical technique you utilize. You should employ the citation style recommended by the
Department, which is Turabian style (see http://library.sonoma.edu/research/turabianstyle.html
for examples).
Some of your public opinion data will be presented in tables. Tables must be included to
document the content of public opinion. All tables have a title in BOLD as well as a source
cited. You must also provide information about the items used in the surveys – question wording,
source of the survey, number of respondents, etc. This information should be included in your
APPENDIX.
LATE PAPERS WILL NOT BE ACCEPTED AND WILL RESULT IN NOT PASSING
THE COURSE. KEEP AN ELECTRONIC COPY of the Final Paper in your own records.
