Discussion: Financing of Health Care

With coinciding concerns about health care costs and the imperative to improve quality of care, health care providers and others face difficult decisions in the effort to achieve an appropriate balance. Such decisions often are addressed in the policy arena. How do policymakers evaluate which health care services should be financed through government programs? How do ethics-related questions and other considerations play into this evaluation process? Is it possible to contain costs and provide accessible, high-quality care to all, or is the tension between cost and care inherent in the U.S. health care delivery system? These questions are central to health care financing decisions in the United States.

For this Discussion, you will focus on the policy decision-making process that determines what types of care are covered by public and private insurers and the ethical aspects of such financial decisions.

To prepare:

Read the case study “Economic Impact of States Declining Medicaid Expansion”  page 190 of the Milstead text( BOOK :HEALTH POLICY AND POLITIC ATTACHED BELLOW) .

Review the information in the Washington Post article “Review of Prostate Cancer Drugs Provenge Renews Medical Cost-Benefit Debate” in the Learning Resources. ( DOCUMENT ATTACHED BELLOW)

Consider how policy decisions currently are made about what will and will not be paid for and what changes, if any, could improve the process.

Reflect on how the Washington Post example illustrates the tension between cost and care.

Post your analysis and assessment of the ethical and economic challenges related to policy decisions such as those presented in the Washington Post article.

How does this type of situation contribute to the tension between cost and care? Substantiate your response with at least two outside resources.

Discussion: Interaction Between Nurse Informaticists And Other Specialists

ASSIGNMENT: Of course, humans don’t fare too badly in this regard either. And healthcare is a great example. As specialists in the collection, access, and application of data, nurse informaticists collaborate with specialists on a regular basis to ensure that appropriate data is available to make decisions and take actions to ensure the general well-being of patients.

In this Discussion, you will reflect on your own observations of and/or experiences with informaticist collaboration. You will also propose strategies for how these collaborative experiences might be improved.

Review the Resources and reflect on the evolution of nursing informatics from a science to a nursing specialty.

Consider your experiences with nurse Informaticists or technology specialists within your healthcare organization.

Assignment: The Impact of Nursing Informatics on Patient Outcomes and Patient Care Efficiencies.

In the Discussion for this module, you considered the interaction of nurse informaticists with other specialists to ensure successful care. How is that success determined?

Patient outcomes and the fulfillment of care goals is one of the major ways that healthcare success is measured. Measuring patient outcomes results in the generation of data that can be used to improve results. Nursing informatics can have a significant part in this process and can help to improve outcomes by improving processes, identifying at-risk patients, and enhancing efficiency.

To Prepare:

  • Review      the concepts of technology application as presented in the Resources.
  • Reflect      on how emerging technologies such as artificial intelligence may help      fortify nursing informatics as a specialty by leading to increased impact      on patient outcomes or patient care efficiencies.

The Assignment: (4-5 pages)

In a 4- to 5-page project proposal written to the leadership of your healthcare organization, propose a nursing informatics project for your organization that you advocate to improve patient outcomes or patient-care efficiency. Your project proposal should include the following:

  • Describe      the project you propose.
  • Identify      the stakeholders impacted by this project.
  • Explain      the patient outcome(s) or patient-care efficiencies this project is aimed      at improving and explain how this improvement would occur. Be specific and      provide examples.
  • Identify      the technologies required to implement this project and explain why.
  • Identify      the project team (by roles) and explain how you would incorporate the      nurse informaticist in the project team.

Comprehensive Integrated Psychiatric Assessment

The comprehensive integrated psychiatric assessment of a child or adolescent consists of gathering information from not only the child but from several sources, most notably the family members, caregivers, and the child’s teacher or school counselor. Because of this, the diagnostic assessment becomes more complicated. Issues of confidentiality, privacy, and consent must be addressed. Also, the PMHNP must take into consideration the impact of culture on the child.

In this Discussion, you review and critique the techniques and methods of a mental health professional as he or she completes a comprehensive integrated psychiatric assessment of an adolescent.

                                               To Prepare for the Assignment:

· Review the Learning Resources concerning the comprehensive integrated psychiatric assessment.

· Watch the Mental Status Examination video.

· Watch the two YMH Bostonvideos.

                                                           Assignment

Based on the YMH Boston Vignette 4 video, post answers to the following questions:

· What did the practitioner do well?

· In what areas can the practitioner improve?

· At this point in the clinical interview, do you have any compelling concerns? If so, what are they?

· What would be your next question, and why?

PLEASE, INCLUDE INTRODUCTION, CONCLUSION, 3 OR MORE REFERENCES LESS THAN 5 YEARS OLD, AND ANSWER ALL THE QUESTIONS AS INSTRUCTED

Organizational Learning And Learning Organizations

Assignment 1—Organizational Learning and Learning Organizations

Organizational learning (OL) refers to situations where, to increase effectiveness, managers work on improving their ability to understand and lead the organization and the task environments. In his book, The Fifth Discipline: The Art and Practice of the Learning Organization, Peter Senge (2006) states that managers can promote organizational learning and create a Learning Organization by:

  • Developing personal mastery.
  • Building complex and challenging mental models.
  • Promoting team learning.
  • Building shared vision.
  • Encouraging systems thinking.

Using the University online library resources, locate a minimum of three different scholarly sources on OL.

Respond to the following:

  • From a manager’s standpoint, discuss the applicability of OL to your current workplace (or to a company you know well) in order to improve its performance.
  • Explain what kinds of actions and changes to the organization need to be implemented to achieve this goal.

Write your initial response in a minimum of 400 words. Apply APA standards to citation of sources.

3. From January 1846 through December 1848 Semmelweis ([1860] 1983) recorded births and the number that resulted in the death of the mother at his hospital. The data is available in the Chapter07.xls file, in the Problem07–03 tab. Create a p chart to analyze performance. Interpret the chart. Was the system stable?

4. Postsurgical infections have been reported to affect 2% to 5% of the 16 million patients who undergo surgery in U.S. hospitals. Infections increase the chance of complications and death. Antibiotics given one hour prior to surgery have been shown to reduce the probabil- ity of infection. The director of quality improvement has sampled 20 patients per week over the preceding 25 weeks. The data is available in the Chapter07.xls file in the Problem07–04 tab. The data collected records whether a patient contracted an infection after surgery. Create a p chart to analyze performance. Interpret the hospital’s performance based on your control chart and identify any issues that should be investigated. Assuming the aver- age rate of infection is 3.5%, is the hospital doing a good job?

5. Readmission rates within one year for congestive heart failure have been documented at 35%. A local heart program wants to assess its performance against this standard. The pro- gram has randomly selected ten patients per month over a 24-month period for review. The data is available in the Chapter07.xls file, in the Problem07–05 tab. Some of the patients were deleted from the sample due to death, relocation, or other reasons that preclude follow-up. Create a p chart. Is the process stable? How is the program performing relative to the documented standard? Since not all months have 10 observations, either use 10 as the sample size or use the average sample size to calculate the control limits.

The essay is intended to depict the bible story of God calling Moses in Exodus 3, 4, and 6. We are to identify what authors wrote which verses and the reason why we think that. Also, we are to redact the passages. Redaction is what occurs when you put both authors and their interpretation of the passages together and how does the general meaning or significance of the bible change. I have attached the file for the instructions for the essay, as well as my class notes. In the notes, I include the general characteristic of the authors J, P, and E and R which means redaction. On the following attachments of the notes, I include several examples the professor gave in class on examples on how to interpret passages from the authors and what happens after redaction. Finally, I attached the book we are to cite as well with the bible passages, which is the book Who Wrote the Bible by Richard Elliot Friedman. The book talks about the authors J, E, and P and the characteristics they have detailing why and how they wrote in the bible. Along with citing the bible passages/verses, we are to cite meaning from the book as well, with evidence from the book how we got to certain conclusions. PROJECT 10 OVERVIEW: RETIREMENT PLANNING
HOW MUCH MONEY WILL I NEED TO RETIRE?
Retirement is the time in life when the major sources of income change from earned income to employer-based retirement benefits, private savings and investments, social security, etc. In this lesson, you will:
• Examine the need to start saving early
• Calculate how much money you will need for retirement
• Identify the sources of retirement income
• Create a strategy for saving for retirement
As you begin your working phase of life, it is a good idea to start planning what you will need to be comfortable during retirement. You will need to start thinking about the following questions:
1. What age do I want to retire?
2. How long do I anticipate living past retirement?
3. What type of lifestyle do I want in retirement?
4. How much money will I need to maintain that lifestyle?
STUDENT HANDOUT 10B
PROJECT 10 ACTIVITY
RETIREMENT PLANNING EXERCISE
STEP 1: ESTIMATE YOUR SOCIAL SECURITY BENEFITS
Using the Social Security Administration’s website, www.ssa.gov/retire2, familiarize yourself with the rules surrounding collecting social security benefits. Determine what age you would like to retire and what your life expectancy might be. If you are currently working or have worked in the past, you can access your personal information by creating an account.
Use this link to open an account and print your benefits statement: https://secure.ssa.gov/RIL/SiView.do.
If you have not had a job or do not have a social security number, use this link to access a sample social security benefits statement: www.ssa.gov/myaccount/SSA-7005-OL.pdf.
Understanding how much you will receive in social security benefits is the first step in planning for your financial future during retirement.
____________ Estimated Monthly Income
____________ Estimated Yearly Income
STEP 2: FIND YOUR NUMBER
Access the retirement calculator here:
http://www.bankrate.com/calculators/retirement/retirement-calculator.aspx.
• Enter how much you think you will need annually in retirement in the first box. To calculate the annual income you will need for retirement, find 80% of your current household income.
• To find the average income for the career path you have chosen (For example, if you want to go into accounting you could use the following link to see what the national average is: www.payscale.com/rcsearch.aspx?category=Job&str=accounting&CountryName=United+States&SourceId=Country.) If you have a different career path, type the job title in the box to the left.
• Recall the chart from the Social Security Administration in Step 1. To find the number of years until retirement, subtract your current age from your anticipated retirement age and enter it in the second box.
STUDENT HANDOUT 10B
PROJECT 10 RETIREMENT PLANNING EXERCISE (CONTINUED)
• To determine how many years you will need income after you retire, , you can look at the average age of relatives. Most experts say that you should plan to live until the age of 90. This may need to be adjusted as you grow older and as life expectancy models change.
• In the annual inflation field, use 3 as your value.
• For the annual yield on balance field, use 5 as your value.
• Click calculate.
• Print the page that shows your number. Put this with your social security benefits estimate.
STEP 3: RETIREMENT INCOME CALCULATOR
Now that you know what you need for retirement, it is important to understand how you will reach such a large number. Use the following link and complete the questions. https://www3.troweprice.com/ric/ricweb/public/ric.do?src=startover
• Do not hit register or log in. Go directly to start.
• This assignment is to be completed as an individual. Do not include a partner or spouse (click no).
• Enter the month and year you were born, click saving for retirement, and then click next.
• In the first box, enter how much you have saved for retirement. If you don’t have a retirement account, indicate zero. If you are not working, assume that you contribute $100 per month to retirement savings.
• Enter your current salary. If you aren’t working, enter the average salary for your chosen career (determined in Step 2, above).
• If you are currently working, enter your correct information into the workplace contributions space. If you are not working, assume that you contribute 5% and your employer matches your 5% contribution for a total of 10%.
• List other contributions if they apply. Click next.
• For asset allocation, use the default settings of 90% Stocks and 10% Bonds. Click next.
• Enter the age you expect to retire.
• Enter how much you think you’ll spend per month in retirement. Recall earlier we indicated that would be 80% of your current household income.
• Click the yes button to include the estimated social security benefit.
• If your place of business provides a pension, include that information in the next box. If you aren’t working, leave it blank. Click next.
• Review the results and print the page.
You will need to turn in the social security benefits sheet, the ING number printout, and the Retirement Income Calculator printout with your Three Legged Stool worksheet.
Be prepared to discuss how you felt about this process.

Post a summary of the interview and a description of the communication techniques you would use with your assigned patient. Explain why you would use these techniques. Identify the risk assessment instrument you selected, and justify why it would be applicable to the selected patient. Provide at least five targeted questions you would ask the patient.

Week 1: Building a Comprehensive Health History

According to a 2011 Gallup poll, nurses are ranked as the most trusted professionals in the United States. One of the most admired nursing skills is the ability to put patients at ease. When patients enter into a healthcare setting, they are often apprehensive about sharing personal health information. Caring nurses can alleviate the hesitance of patients and encourage them to be forthcoming with this information.

The initial health history interview can be an excellent opportunity to develop supportive relationships between patients and nurses. Nurses may employ a variety of communication skills and interview techniques to foster strong bonds with patients and to effectively facilitate the diagnostic process. In conducting interviews, advanced practice nurses must also take into account a range of patient-specific factors that may impact the questions they ask, how they ask those questions, and their complete assessment of the patient’s health.

This week, you will consider how social determinants of health such as age, gender, ethnicity, and environmental situation impact the health and risk assessment of the patients you serve. You will also consider how social determinants of health influence your interview and communication techniques as you work in partnership with a patient to gather data to build an accurate health history.

Learning Objectives

Students will:

  • Analyze communication techniques used to obtain patients’ health histories based upon social determinants of health
  • Analyze health-related risk
  • Apply concepts, theories, and principles related to patient interviewing, diagnostic reasoning, and recording patient information

Discussion: Building a Health History

Effective communication is vital to constructing an accurate and detailed patient history. A patient’s health or illness is influenced by many factors, including age, gender, ethnicity, and environmental setting. As an advanced practice nurse, you must be aware of these factors and tailor your communication techniques accordingly. Doing so will not only help you establish rapport with your patients, but it will also enable you to more effectively gather the information needed to assess your patients’ health risks.

For this Discussion, you will take on the role of a clinician who is building a health history for a particular new patient assigned by your Instructor.

To prepare:

With the information presented in Chapter 1 of Ball et al. in mind, consider the following:

  • By Day 1 of this week, you will be assigned a new patient profile by your Instructor for this Discussion. Note: Please see the “Course Announcements” section of the classroom for your new patient profile assignment.
  • How would your communication and interview techniques for building a health history differ with each patient?
  • How might you target your questions for building a health history based on the patient’s social determinants of health?
  • What risk assessment instruments would be appropriate to use with each patient, or what questions would you ask each patient to assess his or her health risks?
  • Identify any potential health-related risks based upon the patient’s age, gender, ethnicity, or environmental setting that should be taken into consideration.
  • Select one of the risk assessment instruments presented in Chapter 1 or Chapter 5 of the Seidel’s Guide to Physical Examination text, or another tool with which you are familiar, related to your selected patient.
  • Develop at least five targeted questions you would ask your selected patient to assess his or her health risks and begin building a health history.
By Day 3 of Week 1

Post a summary of the interview and a description of the communication techniques you would use with your assigned patient. Explain why you would use these techniques. Identify the risk assessment instrument you selected, and justify why it would be applicable to the selected patient. Provide at least five targeted questions you would ask the patient.

What’s Coming Up in Module 2?

In Module 2, you explore the impact of functional assessments, diversity, and sensitivity in conducting health assessments. You also examine various assessment tools and diagnostic tests used to gather information about patients’ conditions and examine their validity, reliability, and impact in conducting health assessments.

Next week, you will specifically examine functional assessments as they relate to diversity and sensitivity

Registration for Shadow Health

Throughout this course, you will participate in digital clinical experiences using the online simulation tool Shadow Health. The Shadow Health digital clinical experience provides a dynamic, immersive experience designed to improve nursing skills and clinical reasoning through the examination of digital standardized patients. Using Shadow Health you will participate in health histories, focused exams, and a comprehensive assessment.

There will be four Shadow Health assessment components that you will need to complete in Module’s 2 and 3:

  • Health History Assessment (Week 3 & 4)
  • Focused Exam: Cough (Week 5) for a pediatric patient presenting with cough
  • Focused Exam: Chest Pain (Week 7) for an adult patient presenting with chest pain
  • Comprehensive (Head-to-Toe) Physical Assessment (Week 9)

Before you can participate in these simulations, you will need to register for a Shadow Health account. To do this:

  • Go to the Walden Bookstore and purchase access to Shadow Health and the required texts.
  • Once Shadow Health has been purchased, an access code will be emailed to you from the bookstore.
  • Review this video explaining how to register in Shadow Health: https://vimeo.com/275921826/c12d50ee6e
  • Use the Shadow Health link located in the navigation menu on the left in the Blackboard course.
  • Follow the prompts to register in Shadow Health. You will need the access code provided from the bookstore to register. Once registered, Shadow Health should always be accessed via the link in Blackboard.
  • Use only Google Chrome when accessing Shadow Health and make sure all other programs are turned off on your computer. Other browsers do not work well and will not allow the Shadow Health speech to text function to work.
  •  Once registered, complete the Shadow Health Orientation in the Shadow Health website/program and review the videos designed to assist with navigating and completing assignments.
  • Read the Shadow Health Nursing Documentation Tutorial located in the Week 1 Learning Resources.

Note: As nurses you typically use the word assessment to mean completing the physical exam. However, in the SOAP Note format, assessment means diagnosis so start getting in the habit of calling the physical exam exactly that.

Week 2 Case Studies

In Week 2, your Instructor will assign you a case study related to your Discussion by Day 1 of the week. Please make sure to review the “Course Announcements” area of the course to verify your assigned case study. Please plan ahead to ensure you have time to review your case study and your Learning Resources so that you can complete your Discussions and Assignments on time.
