Friendly reminder from professor Friendly reminder: Your written assignment about a plan for either health promotion or disease prevention is due to me by the end of the module, which is in one week from today. The paper does not have to be about the project you are working on for clinical, but it could be. Check in with me if you feel ‘stuck’ or have any questions. I check in to the course and my work email often. Remember to include self-selected scholarly articles to support your work. Here are a few pointers to help you earn the most points! Know clearly what the assignment is asking of you. Read the module overview: “Module 6 Overview: The Health Aggregates”- In Module 1, you identified an aggregate population with which you would be working during this community health course and practicum. The readings and module activities to date have been providing foundational material for use in formulating a problem-oriented plan of care. The activity in this module is a scholarly paper that synthesizes information as a health promotion or disease prevention plan for your target group. (For example, nonveterans living in the community of St. Albans, Jamaica, NY and not having access to use the VA hospital system.) I use this as an example because my previous paper reflected on social determinants of health. I’ll include it…. The social determinants of health that stood out to me in my windshield survey was the lack of healthcare access. “Social determinants of health (SDOH) are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks.” (Healthy People 2030). Considering the area is a residential area with many elderly adults, I feel that there should be more health access available. There is a hospital in the area, but it is only available to veterans. Instead of the gathering hall that was constructed, perhaps it should have been an urgent care instead. An urgent care is a facility or a center that addresses many health issues or ailments. Those who use the centers “felt that their case had been managed with sufficient urgency, that they had contacted the right number of services, and reported their overall care as excellent or very good. (Knowles et. al 2012). The veteran hospital only being available to veterans in the community poses a negative impact to the community. As mentioned, having an urgent care facility available would be beneficial for the community. Next carefully read the instructions for the written assignment itself. If you still have any questions regarding what to do, please reach out to me for clarification. Remember that “disease prevention” and “health promotion” are not necessarily one and the same. Let your plan speak to which of these approaches you are taking. Regarding the components of the paper, follow the directions provided. The component headings are in bold bullet points – this is how you set up your paper; the headings keep your paper organized. Ten pages of content will not be so daunting when you follow this outline, and your content will have meaning. Take the time to review APA writing style and format if you have concerns. What constitutes “scholarly articles”? If you feel that your paper truly benefits from the use of Wikipedia, google search finds, entertainment news, or commercial sites, please be sure to cite them properly. That said, be sure your paper is benefiting from the addition of these sites. They are not considered scholarly sources. A scholarly article is written by experts in their field (look for the credentials after their names, and perhaps an author description). They will have a publishing date. If the article was “peer reviewed”, that means that several experts looked it over before the article was published. You want to look for articles that perhaps include case studies, or analysis of other’s research, or clinical trials. Work with your librarians if you need help or reach out to me anytime. Instructions for paper Compose a scholarly paper that addresses a priority health issue for your target group (aggregate) in their community setting. Write the paper in APA style, using the following guidelines. Include a properly formatted title page and reference page, in addition to 10 pages (+/- 2 pages) of content. Support your content with at least six scholarly references. Components of the Paper: Title Page – see APA Manual (7th ed.) p. 49 and pp. 61-63 for sample Body of the Paper Title of the Paper – Label the introductory paragraph with the title of the paper. This level 1 heading is centered, in title case with a bold font. In describing the community, I did a windshield survey of the community and am including that assignment too in hopes that it will be of great help… Windshield Survey of Jamaica, Queens (St. Albans) The windshield survey of Jamaica, Queens (St. Albans) has allowed me to focus more about the environment, establishments, etc. with a broad perspective. I have been a resident of St. Albans for over twenty-one years. After surveying the area, it makes sense why the establishments that are in the area are in the area, as opposed to other communities. I currently live in long island and found myself comparing the site, which allowed me to explore or survey the community of my selection, St. Albans. Albans is in the southeastern portion of the New York City borough of Queens. St. Albans is centered on the intersection of Linden Boulevard and Farmers Boulevard. It is a residential neighborhood, but there are several businesses in the area, including the facility that I am conducting my community health course. The population consists mostly of African Americans. Hispanics, whites, and other ethnicities/race are at a minimum. Jamaica, Queens (St. Albans) Physical Environment The residential estates from Merrick Blvd & Linden Blvd to Farmers Blvd & Linden Blvd differ. The houses on the west side are more colonial type, as opposed to the east side which contains more multifamily homes, cape cod style, and single-family homes. I cannot emphasize enough how tidy the area is. There are not any homes that appear abandoned. The streets are clean, no excessive trash noted in the street. The Archie Spigner Park can be seen from Linden Blvd. “This park was renamed in 2022 in honor of Archie Spigner (1928-2020), a local politician who served for 27 years as a City Councilman for District 27 in southeast Queens. During his tenure, he advocated for education, infrastructure, and the underserved community.” (nyc.gov) Roy Wilkins Park is located between 115th Avenue and Merrick and Baisley Boulevards. There are many amenities at this facility, it contains baseball, basketball, cricket, handball, swimming, tennis, and track-and-field, as well as a recreation center, fitness equipment, playground, and spray showers. Two medical facilities are situated on Linden Blvd, New York State Veterans Home at St. Albans Nursing and Rehabilitation facility and the New York State VA Hospital and Extended Care. I have been working at this facility for seven years, it is a modern up to date facility (in and out). The VA hospital appears outdated, the building is an orange tinge color, perhaps that is the reason it appears outdated. From the cross street, Mexico and Linden Blvd and Farmers Blvd and Linden Blvd, there are commercial buildings, some are newly constructed, and some are old. The apartments are on top of the commercial buildings. As I emphasized earlier, the area is clean. I have observed two gentlemen cleaning the sidewalk/ raking leaves daily, assisting the garbage men load up the truck, etc. There aren’t factories or signs of pollution observed in the area, but flooding was a major concern to the residents during heavy rain fall. In July 2022, NYC Department of Environmental Protection (DEP) completed a $24 million sewer and water main project to improve street conditions, alleviate flooding and upgrade infrastructure, one year early. “‘The project just completed in South Jamaica and St. Albans is an important investment in our borough’s future,’ said Queens Borough President Donovan Richards Jr. ‘This work underscores the City’s strong commitment to improving street conditions, alleviating flooding, and upgrading infrastructure in all our neighborhoods, especially in southeast Queens. All City residents, no matter where they live, deserve to be served by high-quality infrastructure and to be protected from the life-threatening and property-damaging impacts of flooding. This project goes a long way toward achieving that goal, and full credit should go to the Department of Environmental Protection and the Department of Design and Construction for completing it a year ahead of schedule.’” (Khan 2022). Jamaica, Queens (St. Albans) Economic Considerations Homelessness is common worldwide. The most public areas that homeless individuals seek shelter are on the bus, train station, under the trestle, park benches, bus benches, etc. The entrance to the LIRR -St. Albans station is located on Linden Blvd. Perhaps the time I surveyed the area, I did not notice if homeless people used that area as their shelter. An article mentioned that “some homeless people may respond better to an authority figure like a police officer than to a counselor” (New York Times 1994) when they are asked to relocate or check into a shelter, but I did not notice any police activity or patrolling happening around the area, which allows me to conclude that there is no such activity. The strip of Linden Blvd offers variety of restaurants. There are several bodegas in the area. It will be difficult for anyone to starve. I mentioned earlier that the population is mostly African American, in that instance, the restaurants available offer a lot of Caribbean and southern dishes. There is a Dunkin Donuts, crown fried chicken restaurant and Chinese restaurant available. Luckily there are many bodegas in the area, there is one Key Food Supermarket which is located on Merrick and Linden Blvd. Food stamps are accepted at some bodegas, signs are displayed on the door indicating such. The supermarket definitely accepts it. I noticed that the family dollar department store has a sign inviting applicants to join their team, which signifies that employment opportunities are available in the area. Unemployment rates are at 12.5% in Queens County, according to Healthy people 2030. Jamaica, Queens (St. Albans) Services There are two schools on Linden Blvd. One is located on Merrick and Linden (private school-Allen Christian School) and the other is PS 36 (the entrance is on Foch Blvd, but the rear of the building can be seen on Linden Blvd.) Both schools are modern. PS 36 recently underwent renovation less than a year ago, this resulted in major traffic jams. PS 36 school ranking is considerably low according to the reviews. The reviews praise Allen Christian School for being a great school and scored it 4.1 out of 5 stars. Unfortunately, the library is not that close to where I conducted my windshield survey, but there is a tutoring service available (Kumon). Roy Wilkins Park is a large park that provides various amenities to the community. It is not situated on Linden Blvd, but the park is so large that it can be seen between the cross streets. The Archie Spigner Park can be seen from Linden Blvd. I was surprised to see so many worship facilities, there are a total of four worship vicinities from Merrick Blvd and Linden Blvd to Farmers Blvd and Linden Blvd. One was seven-day Adventist, evangelical church, congregational church, and prayer service facility. The prayer service facility is new, they are open early to anyone who wish to pray for a specific reason, each day is a prayer for a different reason, for instance, Monday’s prayer reflects prayer for the sick, Tuesday is for peace, etc. There aren’t any hospitals in the area for the public. The VA hospital on Linden Blvd is only for veterans. It is a hospital and an extended care/rehabilitation facility. On the same campus is the state nursing home and rehabilitation facility for veterans and the spouses of veterans only. I usually see NYPD school safety patrol vehicles exiting the premises and used this windshield survey as a reason to explore where and why these vehicles were frequently exiting the area and noticed that there is a building across from the VA hospital where they are stationed. After exiting the campus, going east bound, there is a funeral home. I mentioned earlier that I did not notice homeless individuals in the area considering they are likely to seek shelter around the train station, which brings me to the next topic-transportation. The LIRR (St. Albans station entrance is on Linden Blvd), Q4 bus and the dollar van are methods used for transportation in the area. I noticed that traffic is more likely during school hours. Overall, the area is quiet, the people do not seem fearful and feel unsafe. The 113th precinct is not too far, it is located on Baisley Blvd, which is less than 10 minutes away. The fire department is less than 5 minutes away, off Merrick Blvd. Jamaica, Queens (St. Albans) Social Structure Children could participate in martial art activities. I enjoy watching them enter and exit the organization. They are well dressed and ready to fight/interact. I have not seen adults participate, perhaps there is a session for adults, but not when I surveyed the area. There is a small gym for adults a block away. There is a newly constructed event planning hall. I have seen people exit the hall and are well groomed. As mentioned earlier, the population is mostly African American. I have noticed some people entering the LIRR with suit and tie, those waiting for the bus wear the same attire and some appear casual. I cannot recall seeing anyone that appears “gangster.” I think the community, overall, is a safe, quiet, and peaceful community. Conclusion In conclusion, this windshield survey allowed me to take a deeper look at my surroundings and notice organizations/business that I did not know exist. I drive on Linden Blvd daily to go to work, but I would not take notice of the infrastructure of the homes, activities available to people, number of churches on one street. The most alarming discovery is the NYPD school safety building. When I drive to work, after entering the premises, I would bear right until I get to the parking lot of my facility. This survey allowed me to be nosey or investigate/discover why these patrol cars are always exiting this premises too. I lived in this area for so long and did not get the opportunity to use the services provided by this community, because I was not aware or did not notice it.
