Case Management Implementation Plan

Create a plan to implement case management at your workplace.  What is your recommended plan for the use of case managers in your organization for patients with your chosen chronic illness?  Identify people within your organization who are stakeholders or would support your plan.  Whose support do you need to get your plan implemented?  Identify your goals – what do you hope to accomplish with your case management plan?

Often, starting with our goals helps – it’s a backwards design.  As we think about a case management plan we are proposing for our workplace, what do we want to accomplish?  Remember – goals should be measurable.  For example, if the chosen chronic disease is diabetes, one goal may be:

  • Clients will have decreased incidences of hyperglycemia requiring hospitalization

Once we determine what we want to accomplish, we can begin to construct our plan to achieve the goal.  To achieve this goal, our plan may include providing education in a manner the client can understand (being sensitive to cultural needs), access to phone support, or inputting glucose readings into their electronic health record for documentation the case manager can evaluate and use to reach out to the client.

To put this plan in action, whose support would we need?  Primary Care Physicians, Utilization Review personnel, and the Chief Financial Officer would all have an interest in optimizing patient health and reducing costs.

With a clear purpose in place, quantitative researchers have a roadmap for crafting their research questions and hypotheses that will further focus the approach they will take to investigate their topic (i.e., their study’s research design).

The selection of a research design is guided by the study’s purpose and research questions and hypotheses, and the design then links the research questions and hypotheses to the data that will be collected. You should keep in mind, however, that the research process is interactive, not necessarily proceeding in a linear fashion from one component to the next. Rather, the writing of research questions could, for example, necessitate adjustments to the study’s purpose statement. Nevertheless, when presented together, the various components of a research study should align. As you learned last week, alignment means that a research study possesses clear and logical connections among all of its various components.

In addition to considering alignment, when researchers select a research design, they must also consider the ethical implications of their choice, including, for example, what their design selection means for participant recruitment, procedures, and privacy.

For this Discussion, you will evaluate quantitative research questions and hypotheses in assigned journal articles in your discipline and consider the alignment of theory, problem, purpose, research questions and hypotheses, and design. Social norms are defined as the expectations a group or society has regarding situations or individuals within that group or society. In some circumstances this translates to the expectation that older adults need to act old. There are different theories regarding social aging, many of which reflect a particular social norm or even generational perspective on social aging. For example, role theory reflects how a person ages based on his or her perceived roles. According to this theory, a person’s career would often help identify who that person is, such as a doctor, nurse, mechanic, or cook. Another view is the life course perspective, which looks at aging through a longitudinal perspective based on life experiences and normative and non-normative life events.

Tony grew up in an impoverished inner-city community. He was raised by a single mother and had to help with his four younger siblings. As he entered his teen years, the difficulties of living and growing up in his neighborhood began to take their toll. He was skipping school and routinely in trouble with the law. Yet a school guidance counselor recognized Tony’s innate intelligence, his talent as a leader, and his natural charisma, and intervened on his behalf.

Then what happened to Tony? As you might guess, there are many possible outcomes. Here are two extremes: By the time Tony was 20 …

… He had graduated with honors from high school and secured a scholarship to attend college. Since receiving his B.S. in Psychology, Tony has entered graduate school and is working toward an M.S. in Human Resource Management. Tony feels excited and hopeful about his future.

… He had been arrested and convicted of leading a car-theft operation, and went to prison. Since serving his time and being released, Tony has been unsuccessful in finding a job. He feels angry and hopeless about his future.

Consider the clues in Tony’s story about what you will explore as you examine social-cognitive theories of personality. What role has Tony played in how his life is turning out and how he thinks about his future? What are social influences on Tony, such as where he grew up and the guidance counselor who intervenes? How might a social-cognitive theorist explain these very different outcomes for Tony?

Your work in this final week of the course will offer answers. You will consider distinguishing features of social-cognitive theories of personality and how social origins of behavior and cognitive—or thought—processes can shape a person’s development.

Discussion: Testing Reciprocal Determinism

Recall the story of Tony from the Week 6 introduction and consider the principle of reciprocal determinism, which was discussed in the reading. Your course text defines the term as “the mutual, back-and-forth effects of variables on one another; in social-cognitive theory, a fundamental causal principle in which personal, environmental, and behavioral factors are viewed as causally influencing one another” (Cervone & Pervin, 2019, p. 424). How might different personal, environmental, and behavioral factors have causally influenced both the positive and negative outcomes for Tony?

Naturally, it is difficult to answer that question about Tony because you have only so much information about him. However, someone that you know a great deal about is yourself and your own experiences. For this Discussion, you will explore the concept of reciprocal determinism by applying it to an experience in your own life.

To prepare:

  • Review the reading in the course text this week, paying particular attention to the explanation of reciprocal determinism on pp. 344 and 345.
  • Consider a personal experience you feel comfortable sharing with colleagues that you could analyze from the perspective of reciprocal determinism.

Research Project: Analyzing Effects of Media on Behavior

Practically from the advent of television, there has been a persistent question about its influence on viewers, particularly young people. With the introduction of video games, that question was transferred to yet another form of media. You may have your own personal views about television and video games, but for this Assignment—or at least to start—set those views aside as you assume the role of a researcher analyzing effects of media on behavior.

You will write a research paper in which you apply theories of behavior modeling to explain positive or negative impacts of television and other media viewing on behavior.

To prepare:

  • Review the Week 6 Learning Resources with information on behavior modeling. Be sure to access the Handout with suggested resources that address television and video-game viewing and their influence on behavior.
  • Choose a perspective for your research paper. You will discuss and provide evidence for the potential positive influences of behavior modeling from television and other media viewing or the potential negative influence on behavior.
  • As part of your research, consider practical recommendations to include in your paper for how to strengthen positive influences of television/media viewing or how to lessen the negative influences on behavior.
  • Be aware that you must use three resources to inform your paper—the course text and two other resources. You may choose from the resources listed in the Handout or resources that you find.

Submit a 2- to 3-page paper that addresses the following:

Identify your position on the influence of television and other media viewing on behavior.
